Summary of Entrance Requirements

This information is also included in the EMT Program Packet. In order to enter the EMT Program, you must meet the following requirements:

    1. Register for the course;
    2. Pay the registration fee and select a payment plan;
    3. Hold a High School Diploma, Home School Diploma, or General Education Degree. Students will need to send a copy of their High School Diploma or GED or send official transcripts showing that he/she graduated;
    4. Register for the Assessment Exam (Free): or demonstrate proof of college study as outlined below. Use the table below to determine the recommended/required courses as the table represents the recommended and required courses to take in relation to the assessment score obtained. Note, this requirement is waived if the student has an Associates Degree or higher, has previously completed a state recognized EMT training course with a grade of "C" or higher within the past 5 year, or at least 2 years of full-time study or the equivalent from an accredited college or university with a cumulative GPA of 2.0 or higher. You may submit unofficial transcripts prior to the start of class to satisfy this requirement, provided that official transcripts are sent as proof within 30 days after the class start date.

    5. The applicant must be eighteen years of age or older at the time he or she starts his or her program of study. Participants who are less than 18 years of age at the time of registration, must provide parental approval of all written and electronic forms.
    6. Accepted applicants are bound to the agreement contained on the registration form.
    7. Proficiency in cardiopulmonary resuscitation (Not Expiring within the next 6 months, taken with a LIVE instructor), demonstrated by one of the following:
    8. The applicant must demonstrate proficiency in reading at the 9th grade level. This will be verified through the presence of a High School Diploma or GED.
    9. Negative Drug Screening Test - not required prior to entry, but required prior to externship participation. (must be 5 panel or greater and completed within the last 3 months). Students can arrange for a drug screening at Results Testing;
    10. Negative TB Test which may include either a PPD or chest X-ray with negative indication. Students can arrange for a TB Test at Results Testing.You can also visit Any Lab Test Now or call (602) 955-0240.
    11. MMR vaccination or immunity. not required prior to entry, but required prior to externship participation. Students can arrange for an MMR vaccination at Results Testing. You can also visit Any Lab Test Now or call (602) 955-0240.
    12. Varicella vaccination or immunity. not required prior to entry, but required prior to externship participation. Students can arrange for an Varicella vacinnation at Results Testing. You can also visit Any Lab Test Now or call (602) 955-0240.
    13. Tetanus, Diphtheria, and Pertussis vaccination within the last 10 years or immunity. not required prior to entry, but required prior to externship participation. Students can arrange for a TDAP vacinnation at Results Testing. You can also visit Any Lab Test Now or call (602) 955-0240.
    14. Hepatitis B vaccination, immunity, or signed Hepatitis B Declination Form. not required prior to entry, but required prior to externship participation. Students can arrange for an Hepatitis B vacinnation at Results Testing. You can also visit Any Lab Test Now or call (602) 955-0240.
    15. Flu vaccination for the current season or waiver. Note, the waiver may not be available in all externship locations or may require the student to meet other restrictions/requirements, such as wearing a mask during time spend in the hospital, etc... not required prior to entry, but required prior to externship participation.     16. DPS Fingerprint clearance Card - not required prior to entry, but required prior to externship participation. Fingerprinting services will be offered on the first day of the class at 5PM for $12.50 per student. Obtaining a fingerprint clearance card takes 6+ weeks. Therefore, it is helpful to complete this step prior to entry into the program. The cost varies (between $10-$25) for the fingerprinting (payable on the day of service) and $65 for the application to the Arizona Department of Public Safety.     17. Note: If the student is unable to complete externships as scheduled due to not obtaining a fingerprint clearance card or not obtaining labs or a drug test, they may be dismissed from the program administratively.
